2025年广东暴发期间基孔肯雅病毒ECSA-MAL谱系的病毒动力学研究

  • 本研究通过对2025年广东省基孔肯雅热(CHIKF)暴发期间采集的样本进行分析,探讨了基孔肯雅病毒(CHIKV)ECSA-MAL 谱系的病毒动力学特征。研究共纳入1146份发病1-4天内的横断面静脉血样本和37份纵向毛细血管血(指尖血)样本。结果显示,静脉血和指尖血组在发病初期具有较高的病毒载量均值,且在发病1-4天内呈现显著的下降趋势(P < 0.001)。通过对指尖血样本的连续监测发现,病毒载量在发病5-7天内降至检测限以下。静脉血清的检测灵敏度通常比指尖血高,但在发病1~3天的病毒血症高峰期,指尖血病毒载量远高于检测限,诊断效能与静脉血无差异;且指尖采血为微创操作,无需专业采血人员,可支持快速分子筛查。年龄亚组分析表明,0-17岁儿童组的病毒清除最为迅速,而61岁以上老年组的病毒清除效率相对较低,维持了较长时间的高病毒载量水平。本研究首次阐明了 ECSA-MAL 谱系在人群中的快速复制与清除规律,为疫情监测和防控提供了重要的科学依据。
